基底节区腔隙性脑梗死治疗前后MR表现分析

摘    要:目的研究基底节区腔隙性脑梗死临床治疗前、后的生理及病理特征。方法收集28例基底节区腔隙性脑梗死患者病例资料,以入院治疗前患者的头颅核磁共振(MR)图像数据作为治疗前组,入院治疗一定时间后复查头颅MR所得数据作为治疗后组,对比治疗前后两组病灶范围大小及信号高低的变化。结果治疗后大部患者(男性87.5%,女性83.3%)原发病灶趋于稳定,少数患者病灶(男性12.5%,女性16.7%)扩大趋势。通过MR检查,发现急性期发病并及时诊断及治疗的患者,病情得到很好控制。结论 MR对腔隙性梗死诊断有很强的敏感性,对腔隙性脑梗死患者周边缺血半暗带也有很清晰的显示,急性期诊断符合率高,可作为诊断腔隙性脑梗死及是否治疗有效的重要指标之一。

Abstract:Objective To research the pathological characteristics of lacunar cerebral infarction in basal ganglia region on MR images before and after treatment. Methods The clinical data of 28 cases of acunar cerebral infarction in basal ganglia region was collected. Their imaging data of encephalic nuclear magnetic resonance before the treatment was compared with that after the treatment.The changes in area of lesion and magnitude of MR signal were observed and compared between the two groups. Results After treatment,the primary lesions became stable in most patients( 87. 5% male and 83. 3% female) and became bigger in a few ones( 12.5% male and 16.7% female). Those patients who were in acute stage and diagnosed and treated in time had a good control of the disease conditions. Conclusions MR has a strong sensitive to the diagnosis of acunar cerebral infarction and can demonstrate very clearly the peripheral ischemic semidark area around the lesion. It has a high diagnose accordance rate with this disease in acute stage and can be used as an important index of diagnosis of acunar cerebral infarction in basal ganglia.
