肿瘤型髋关节假体治疗高龄粗隆间粉碎性骨折的疗效研究

目的回顾应用肿瘤型髋关节假体置换治疗高龄股骨粗隆间粉碎性骨折的疗效。方法选择2008年1月～2011年12月于我科接受治疗的35例高龄股骨粗隆间粉碎性骨折患者,其中男性9例,女性26例,平均81.6岁,均接受肿瘤型人工髋关节置换治疗。术后3 d开始进行床上功能锻炼。术后6个月开始随访。随访时根据术后并发症、Harris评分及患者生活质量评价疗效。结果平均手术时间54 min,术中平均出血量250 ml。所有患者均得到随访,平均随访时间13.6（6～24）个月,所有患者均恢复良好；根据Harris评分,优17例,良14例,可4例,优良率88.6%。结论应用肿瘤型人工髋关节置换治疗高龄股骨粗隆间粉碎性骨折效果满意,该方法值得推广应用。

Curative effect of tumor type hip prosthesis replacement on the senile patients with comminuted intertrochanteric fracture of femur

Objective To review the curative effect of tumor type hip prosthesis replacement on the senile patients with comminuted intertrochanteric fracture of femur. Methods 35 senile patients with comminuted intertrochanteric fracture of femur selected for the research from Jan.,2008 to Dec.,2011; 9 males and 26 females with the mean age of 81. 6 years,underwent the tumor type hip prosthesis replacement; functional exercises in bed started 3 days after operation and follow-up lasted 6 months after operation;the curative effect was evaluated based on the post-operative occurrence of complications,Harris scores and quality of life. Results The average operation time was 54 minutes and the average bleeding volume was 250 ml; all the patients were followed up for a mean duration of 13. 6 months; all recovered,and according to Harris score,excellent in 17 cases,good in 14 cases and acceptable in 4 cases,the excellent and good rate was 88. 6%. Conclusions The tumor type hip prosthesis replacement is of satisfactory effect on the senile patients with comminuted intertrochanteric fracture of femur,and the technique is worthy of wider application.
