选择性非手术治疗脾破裂27例临床观察

目的探讨腹部闭合性损伤脾破裂非手术治疗的观察体会。方法选择27例2009年1月～2010年12月本院收治的选择性非手术治疗脾破裂患者的临床资料进行回顾性分析。结果 27例患者中,有26例均经保守治疗,住院10～22d均痊愈出院,1例住院后第4天因出现延迟性脾破裂（DRS）而中转手术。所有患者出院3个月后经CT复查显示脾破裂已愈合,经半年～2年随访,患者均无异常。结论选择性非手术治疗脾破裂与其他保脾疗法相比较,具有无创伤、患者痛苦小、避免手术及其带来的并发症等优点。

Clinical observation on selective non-surgical in the treatment of splenic rupture in 27 cases

Objective To explore the observation experience of non-surgical treatment for blunt abdominal trauma rupture of spleen.Methods Twenty-seven clinical data of patients with splenic rupture who treated with selective non-surgica in our hospital from January 2009 to December 2010 were retrospectively analyzed.Results Among the 27 cases of patients,with the exception of one transferred to surgery after the delayed splenic rupture （DRS） at the fourth day in our hospital,other 26 cases were discharged 10 to 22 days later by conservative treatment.CT scan was taken at three months later confirmed splenic rupture was healed and patients showed no abnormality during next 6 months to 2 years follow-up.Conclusion Compared with other non-invasive therapy,non-surgical treatment of splenic rupture have the following advan tages：no wound,less pain,avoiding surgery and the complications induced by the surgery.
