氟比洛芬酯联合芬太尼与单用芬太尼用于上腹部手术术后静脉自控镇痛的临床比较

目的通过比较氟比洛芬酯联合芬太尼与单用芬太尼用于上腹部手术后患者,观察术后静脉自控镇痛（PCIA）的镇痛强度、镇静效果及不良反应。方法选择上腹部开腹手术术后行PCIA患者30例。随机均分为观察组和对照组,每组各15例。观察组于手术结束前30min给予氟比洛芬酯50mg,术后镇痛给予氟比洛芬酯200mg＋芬太尼0.5mg＋0.9%氯化钠溶液共100mL;对照组术后镇痛给予芬太尼1.0mg＋0.9%氯化钠溶液共100mL。PCA泵2mL/h,锁定时间15min。观察两组术后48h内（6、12、24、48h）的镇痛镇静评分及不良反应发生情况。结果两组术后镇痛及镇静评分比较,差异无统计学意义（P〉0.05）,观察组不良反应发生率低于对照组。结论氟比洛芬酯联合芬太尼用于上腹部手术后的PCIA效果良好,且能减少芬太尼用量,同时降低不良反应如恶心、晕眩、呕吐的发生。

Clinical comparison of Fiurbiprofen Axetil combined with Fentanil and Fentanil alone for postoperative intravenous analgesia for upper abdominal surgery

Objective To compare Flurbiprofen Axetil combined with Fentanyl and Fentanyl alone for upper abdominal surgery patients,and the intensity of postoperative intravenous analgesia （PCIA） of analgesic,sedative effects and adverse reactions were observed.Methods Thrity cases with upper abdominal surgery were selected and randomly allocated into observation group and control group,each group had 15 cases.In the observation group,the patients were given Fiurbiprofen Axetil 50 mg in 30 min before the end of surgery,and Fiurbiprofen Axetil 200 mg plus Fentanil 0.5 mg plus Saline a total of 100 mL in postoperative analgesia.In the control group,the patients were given Fentanil 1.0 mg plus normal Saline a total of 100 mL.PCA pump 2 mL/h,locking time 15 min.The analgesic and sedation scores and the incidence of adverse reaction in two groups were observed after 48 hours （6,12,24,48h）.Results The analgesic and sedation scores between two groups were no significant different （P 0.05）.The incidence of adverse reactions in observed group lower than that of the control group.Conclusion Fiurbiprofen Axetil combined with Fentanil for upper abdominal surgery has good effect,and reduce the dosage of Fentanil,reduce adverse effects such as nausea,dizziness,vomiting.
