异丙酚复合氯胺酮或芬太尼静脉麻醉在胃镜检查中的应用

目的观察异丙酚复合芬太尼或氯胺酮静脉麻醉用于胃镜检查的效果。方法90例行胃镜检查患者随机分为三组：异丙酚组（P组），异丙酚复合芬太尼组（F组）或氯胺酮组（K组）静脉麻醉，每组30例。观察各组异丙酚用量、意识消失时间、停药后清醒时间、清醒后独立离院时间；监测BP、HR、SpO2、术中及术后的不良反应。结果异丙酚用量P组显著多于K、F组。K组意识消失时间短（P〈0．05）；F组清醒时间和离院时间长（P〈0．05）。F组SpO2下降幅度及下降率高（P〈0．01）；P组和F组低血压、心率减慢发生率高于K组（P〈0．05、P〈0．01）。F组术后站立眩晕率高（P〈0．05）。结论异丙酚复合氯胺酮静脉麻醉与单纯异丙酚或复合芬太尼相比，诱导和清醒更加迅速，呼吸循环抑制轻，是胃镜检查安全可行的麻醉方式。

The apply of propofol intravenous anesthesia with fentanyl,kentamine in endoscopy examine

Objective To observe the effect of propofol anesthesia with fentanyl and kentamine in gastroscope examination.Methods Ninety patiens were randomly divided into three groups with 30 cases each:propofol intravenous anesthesia(group P),and compound fentanyl(group F) or kentamine(group K) separately.The propofol dosage,consciousness loss time,wake-up time,independenly leave hospital time were recorded.Observing BP,HR,SpO_2 and side effects during intro-and post-operative.Results The dosage of propofol was larger in group P than other groups(P<0.05),consciousness loss time in group K was shorter(P<0.05),wake-up time and independenly leave hospital time were longer in group F than that in the others(P<0.05).SpO_2 down range and rate were significantly higher in group F(P<0.01).BP and HR decrease were significantly higher in group P and F(P<0.05,P<0.01).Postoperative vertigo was higher in group F(P<0.05).Conclusion Propofol-kentamine anesthesia are rapider in inducement and recovery without significant side effects,and could be a better choice for gastroscope examintion.
