彩色多普勒超声对肾移植患者手术前后多脏器功能的评价

摘    要:目的 探讨肾移植术前后应用彩色多普勒超声对患者心肾功能的动态观察的应用价值.方法 应用彩色多普勒超声对152例肾移植病例进行术前评价和术后监测.肾移植术前后观察肾脏、输尿管、膀胱及心脏的功能情况,重点观察肾移植术后3 d、7 d、1个月及术后3个月内移植肾形态结构及各项血流参数指标.结果 术后3个月超声心功图检查,正常组的各心腔均有所缩小,室间隔和室壁搏幅增加,厚度变薄,心功能有所改善;术后多普勒超声及时发现152例肾移植病例中出现手术并发症64例,发病率为42.11%.其中急性排斥反应23例,吻合口处肾动脉狭窄14例,肾盂积水25例,其中导致输尿管扩张18例,肾周积液20例.结论 应用彩色多普勒超声动态观察肾移植术前后泌尿系、心脏功能情况以及移植肾内的血流灌注情况,对各项血流参数指标进行临床评估,有助于及早发现术后并发症,对于提高肾移植术的成功率和患者的生存率具有良好价值.

Abstract:Objective To assess the applied value of color Doppler ultrasound in continuely monitoring the function of multi-viscus fore-and-aft kidney transplantation.Methods The ultrasonic features of one hundred and fifty-two cases with kidney transplantation were analysed,and the complications of kidney transplantation were observed emphasisly three days, seven days, one month and three months after operation.Results After three months of operation,every chambers heart of the normal group have deflated,IVS and chamber wall have attenuated, and the heart function have improved too.There were 64 technical complications in our series with an incidence of 42.11%. It included 23 cases of acute rejection, 14 cases of kidney artery stenosis, 25 cases of hydronephrosis,among 18 cases of ureterectasia and 20 cases of peritransplant fluid collection.Conclusion Color Doppler ultrasound is a useful tool for early diagnosis of kidney transplant complications, it also can improve the kidney transplantation and patient survival rates successfully.
