早发LH峰与“抢收”

众所周知，女性生殖功能的维持有赖于正常的下丘脑-垂体-卵巢轴的激素分泌以及完整的反馈调节机制，其中黄体生成素（LH）在控制和调节卵泡的发育成熟以及排卵和黄体形成及其功能维持等方面发挥着重要作用：在自然周期，LH不仅能在月经中期促发排卵，促进卵母细胞恢复成熟分裂，而且在雌激素的分泌、优势卵泡的选择以及卵母细胞的成熟等诸多方面发挥着重要作用。

Premature LH surge and "rush harvesting"

This paper reviews the reason and the prevalence of premature LH surge in controlled ovarian hyperstimulation （COH） and its possible adverse effects on IVF outcomes. According to the literatures and our preliminary study, rush harvesting may be a cost-effective and more practical way in the occurrence of premature LH surge. It is outlined as follow： when serum progesterone level 〈2. 0 ng/ml （6.34 nmol/L） before hCG administration, hCG injection, oocyte retrieval and embryo transfer should be performed routinely; If serum progesterone level ≥2.0 ng/ml, embryo transfer should be cancelled and embryos cryopreserved; If serum progesterone level ≥ 2. 0 ng/ml and serum estradiol level decreased significantly compared with the previous day （≥30 %）, an immediate injection of hCG should be performed and ooeyte retrieved 24 hours after hCG administration, then fresh embryo transfer cancelled and embryos cryopreserved.
