拉西地平治疗高血压对BNP及左室肥厚的影响研究

目的探讨血清B型脑钠利肽(BNP)与高血压左室肥厚的相关性,并观察拉西地平治疗对LVH和BNP水平的影响。方法轻、中度高血压患者150例,根据左室质量指数(LVMI)分为左室肥厚组(LVH)和无左室肥厚组(NLVH),给予拉西地平治疗6个月,观察血清BNP水平和LVHI治疗前后及组间变化,并与健康对照组对比。结果拉西地平治疗前,LVH组和NLVH组血清BNP水平高于对照组(P<0.01),并LVH组高于NLVH组(P<0.01);治疗后LVH组LVMI、血清BNP水平均有显著下降(P<0.01);NLVH组治疗后仅LVMI较治疗前有明显下降(P<0.05)。血清BNP水平与BNP和LVMI均呈正相关(r=0.31,P<0.05,r=0.45,P<0.01)。结论拉西地平不仅可以平稳降低血压,还可逆转高血压伴左心室肥厚。

The Influence of Lacidipine on Serum Level of BNP and Myocardial Wall Motion in Hypertension

Objective To explore the relation between brain natriuretic peptide（BNP）with left ventricular hypertrophy（LVH）induced by hypertension,and to observe the therapeutic effect of lacidipine.Methods One hundred and fifty mild and moderate hypertension patients enrolled the study,and were divided into LVH group and non-LVH group（NLVH）.All patients were administrated lacidipine for 6 months.The serum BNP and LVMI were compared pre and post therapy,and compared with 40 healthy subjects,with served as control group.Results Before lacidipine treatment,the serum BNP and LVMI were higher in LVH and NLVH hypertension patients than those in control group（P〈0.01）,and the indexes in LVH group and higher than those in NLVH group（P〈0.01）.After lacidipine treatment,the levels of serum BNP reduced in LVH group（P〈0.01）.There was different result in NLVH group,which only obviously decreased in LVMI（P〈0.05）.It was shown that blood pressure and level of blood serum BNP or LVMI had positive correlation（r=0.31,P〈0.05;r=0.45,P〈0.01）.Conclusion The results indicate that the lacidipine could drop the blood pressure,also revert the left ventricular hypertrophy in hypertension patients.
