摘    要:目的:探讨磁共振胰胆管造影术(MRCP)、弥散加权序列(DWI)对胆道梗阻性疾病的应用价值。方法:采用重T2加权水成像技术和弥散加权序列对50例患者行MRCP、DWI检查,MRCP诊断结果与病理结果进行对照分析;另选10名健康志愿者作为对照组,然后按照实验室胆红素水平将50名患者分为轻中重组,将组间ADC值进行对比分析。结果:MRCP对胆道梗阻性疾病的定位诊断准确率为96%(48/50),定性诊断准确率为84%(42/50)。结论:ADC值能够反映肝脏损伤程度、评价肝脏功能,MRCP联合DWI序列扫描对胆道梗阻性疾病起到定位定性诊断的作用且准确率高,可为临床治疗提供可靠依据。

Clinical application of magnetic resonance cholangio-pancreatography in obstructive jaundice

Abstract:Objective:To study the diagnostic value of MR Cholangiopancreatography(MRCP) and Diffusion-weighted Imaging(DWI) in biliary tract obstructive diseases.Methods: 60 patients with poststroke hemiplegia from June 2008 to Mar 2011 were randomly divided into control group(n=30) and observe group(n=30).Patients in the control group were given conventional treatments,and patients in the observe group were given body weight support treadmill training.After 12 weeks of treatment,motor ability,10 meters walking time,motor function of crura and activity of daily life(ADL) were analyzed.Fifty obstructive jaundice patients were studied by using MRCP and DWI, then comparing MRCP diagnosis and pathological results.Another 10 healthy volunteers as a control group,and fifty patients were divided into three-grade scale by the content of serum bilirubin in serum,then ADC values between different case-groups were measured.Results: The diagnostic accuracy of MRCP displaying obstructive was 96%(48/50) with a qualitative diagnostic accuracy of 84%(42/50).Conclusion: ADC values can reflect the degree of liver lesion and be significative to the evaluation of liver function,and MRCP combined with DWI can display accurately the location and the nature of obstructive jaundice,which provides the reliable basis for clinical treatment.
