假瘤样脱髓鞘病的临床特征(附4例分析)

目的探讨假瘤样脱髓鞘病的临床特征。方法回顾性分析4例假瘤样脱髓鞘病患者的临床资料。结果 4例患者以急性或亚急性起病为主,影像学表现有占位效应和强化征,经类固醇激素冲击治疗后症状和体征好转。随访后2例为多发性硬化,1例为急性播散性脑脊髓炎。结论假瘤样脱髓鞘病临床特征与颅内肿瘤类似,在影像学上有一定的特征性表现,可能是多发性硬化和急性播散性脑脊髓炎的早期过渡阶段,早期正确的诊断结合类固醇激素治疗可获得满意疗效。

Clinical characteristics of pseudotumor forms of demyelinating disease:a report of four cases

Objective To discuss the clinical characteristics of pseudotumor forms of demyelinating disease.Methods Four cases of pseudotumor forms of demyelinating disease were retrospectively analyzed,including of clinical features,imaging and therapy.Results Four cases were acute or subacute anset,the neuroimaging showed the lesions around with space-occupying effect which could be enhanced.All four patients were relieved significantly with corticosteroid therapy.Two patients relapsed after treatment which diagnosed as multiple sclerosis(MS),one patient who was affirmed acute disseminated encephalomyelitis(ADEM)evolved.Conclusion The feature of pseudotumor forms of demyelinating disease is similar to some extent intracranial tumors,however it has own radiological characteristics.It's may an intermediate entity between MS and ADEM.Diagnostic cortieoid therapy may be helpful to draw a diagnosis
