甲状腺手术中甲状腺上动脉的判断与处理技巧

目的研究甲状腺手术中甲状腺上动脉的判断与处理方法。方法56例甲状腺手术中,用小纱布球紧贴上极内侧真被膜推移剥离后,见到沿甲状腺上极内侧缘走行的条索状白色细小血管,有一定弹性,顺延该动脉向上既可找到甲状腺上动脉腺支前支。紧贴腺体三重结扎后切断,同样找到后支切断。结果56例中,1例在分离上极时有明显出血约30ml,其余患者出血量不足5ml,视野清晰,上极血管显露清楚结扎牢靠。全部患者均无喉上神经损伤表现。结论紧贴上极内侧甲状腺真被膜剥离,首先寻找甲状腺上极内侧缘走行的小分支,顺势而上必定能准确找到甲状腺上动脉腺支前支。该方法可以准确识别甲状腺上血管,处理安全方便。

The technique of management of superior thyroid artery in thyroidectomy

Objective To study the method of dissection of superior thyroid artery in carrying out thyroidectomy. Methods 56 cases were nsuded, along the internal margin of the upper thyroid pole and close to its capsule, all the connective fibrous tissues were dissection. The branch trunk of superior thyroid artery（STA） were always identified. Results Of all the 56 cases , all the STA were successfully identified, no one experienced postoperatively superior larynx nerve damagement. Conclusion This technique plays a key role in ensuring the identification of STA in thyroidectomy.
