非酒精性脂肪性肝病患者中医证型与胰岛素抵抗指数、脂联素/瘦素比值的相关性研究

目的：探讨非酒精性脂肪性肝病（NAFLD）患者中医证型与胰岛素抵抗指数（HOMA—IR）、脂联素／瘦素比值（APN／LEP）的相关性。方法：对186例NAFLD患者辨证分为肝郁湿阻型、湿热蕴结型、痰瘀互结型和脾肾阳虚型，均进行空腹血糖（FPG）、胰岛素（FINS）、瘦素（LEP）和脂联素（APN）检测，比较HOMA—IR和APN／LEP在各组之间的差异。结果：痰瘀互结型和脾肾阳虚型患者HOMA-IR显著高于肝郁湿阻和湿热蕴结型（P〈0．05）；湿热蕴结和痰瘀互结型患者APN／LEP显著低于肝郁湿阻和脾肾阳虚型（P〈0．05）。结论：综合HOMA—IR和APN／LEP对于判定NAFLD的中医证型有一定的应用价值。

Correlations between TCM syndrome type and insulin resistance index, adiponectin leptin ratio in patients with nonalcoholic fatty liver disease

Objective： To investigate the correlations between TCM syndrome type and insulin resistance index, adiponectin leptin ratio in patients with nonalcoholic fatty liver disease （NAFLD） . Methods： The syndromes of 186 NAFLD patients were divided into Ganqi stagnation and dampness accumulation type, dampness heat accumulation type, phlegm blood stasis type and Yang-deficiency of both spleen and kidney type. The levels of fasting plasma glucose （FPG）, insulin （FINS）, leptin （LEP） and adiponectin （APN） were detection, and the differences between HOMA-IR and APN / LEP in each type were compared with. Results： The HOMA-IR in phlegm blood stasis type and Yang-deficiency of both spleen and kidney type was significantly higher than other groups （ P 〈 0.05 ） . The APN/LEP of dampness heat accumulation type and phlegm blood stasis type were significantly lower than other type （P 〈 0. 05） . Conclusion： The integration of HOMA-IR and APN/LEP has a certain application value for TCM syndrome type of decision NAFLD.
