心脏直视手术315例临床分析

目的:分析315例心脏直视手术的体会.方法:总结315例心脏直视手术,分析各种常见的手术并发症,探讨减少手术并发症的措施.结果:术后出现各种心律失常95例,呼吸系统感染41例,泌尿系统感染11例,胸腔积液8例,心包积液4例,低心排综合症6例,关胸后心跳骤停又紧急开胸按压2例,急性肾功能衰竭2例,脑栓塞2例,灌注肺1例,死亡6例.结论:心律失常和呼吸系统感染是心脏最常见的并发症:重视术前心肺功能的调整;术中维持有效的组织灌注压,注重心肌及各器官功能的保护;加强术后呼吸道管理,能明显减少术后并发症.

Clinical analysis of 315 cases with open-heart surgery

Objective To analyze the clinical experience of 315 cases with open-heart surgery. Methods To summarize miscellaneous postoperative complications in 315 cases with open-heart and discuss measures of reducing complications after operation. Results Of them, arrhythmia cases were 95, relatively , respiratory infection were 41, urinary infection were 11, pleural effusion were 8 , pericardial effusion were 4 , syndrome of low cardiac output were 6 , cardiac arrest were 2, acute renal failure were 2 , cerebral embolism were 2, perfusion lung was 1, fatality were 6. Conclusion Arrhythmia and respiratory infection is one of the most frequent postoperative complications. Improvement of preoperative cardiopulmonary function and sustaining the valid perfusion pressure can to reduce incidence of complications. Protecting myocardium and multiple organ function in operation and enhancing the management of respiratory system after operation can obviously reduce complications in patients undergoing open-heart operation.
