慢性乙型肝炎患者血清免疫球蛋白水平的变化及临床意义

目的探讨慢性乙肝患者血清免疫球蛋白水平变化及其临床意义。方法采用免疫比浊法对我院2010年1月至2012年1月收治的100例乙型肝炎患者其中慢性乙肝（CHB）患者53例、慢性重型乙肝（SHB）患者47例]及同期50例健康体检者（健康对照组）的血清免疫球蛋白（IgG、IgA和IgM）进行检测,比较各组患者免疫球蛋白水平。结果 CHB组与SHB组患者IgG、IgA和IgM水平均明显高于健康对照组,且SHB组免疫蛋白水平明显高于CHB组,差异均具有统计学意义（P〈0.05）;病情改善组IgG、IgA和IgM水平均明显低于恶化组,且两组均较治疗前显著变化,差异均具有统计学意义（P〈0.05）。结论血清免疫球蛋白水平能反映慢性乙肝患者肝细胞受损程度,可为临床诊断、治疗和预后提供检测依据。

Changes of serum immunoglobniin levels in patients with chronic hepatitis B and their clinical significance.

Objective To investigate the changes of serum immunoglobulin levels in patients with chronic hepatitis B and their clinical significance. Methods The serum immunoglobulin （IgG, IgA, and IgM） of 53 chronic hepatitis B patients （CHB group）, 47 chronic severe hepatitis B patients （SHB group） and 50 healthy individuals （the control group） in our hospital from January 2010 to 2012 January were detected, and the immunoglobulin levels were compared between the three groups. Results IgG, IgA and IgM levels of CHB group and SHB group were signifi- cantly higher than those of the control group, and the immune proteins of SHB group was significantly higher than those of CHB group, all with statistically significant difference （P〈0.05）. The IgG, IgA and IgM levels were signifi- cantly lower in the patients with improvement than those with deterioration, and the difference was statistically signifi- cant （P〈0.05）. Conclusion Serum immunoglohulin levels in patients with chronic hepatitis B can reflect the degree of liver cell damage, which provides reference for the clinical diagnosis, treatment and prognosis.
