股骨远端C型骨折手术疗效的相关因素分析

目的分析股骨远端C型骨折的手术效果，探讨提高疗效的相关因素。方法对2001年7月～2004年12月22例股骨远端C型骨折，给予切开复位、AO股骨髁支持钢板及AO动力髁螺钉（DCS）内固定治疗，术后给予必要的对症治疗与合理的功能锻炼。结果随访1224个月，平均16个月，按Neer膝关节功能评定标准评价，22例患者，优6例，良12例，中2例，差2例，优良率达81．8％。结论严格遵循手术治疗原则，争取早期手术；依据分型，选择适当内固定方法；塌陷骨折复位，恢复关节面平整和坚强内固定；控制并发症；术后早期不负重适度功能锻炼等可提高疗效。

Factors relative to surgical treatment of type C distel femoral fracture

Objective To analyze the clinical effect and relative factors of surgical treatment of type C distal femoral frecture. Methods From July 2001 to December 2004,22 patients with type C distal femoral fractures were treated with the open reduction and internal fixation.The supporting plate of AO femoral condyles were applied in 10 cases,DSC in 12 cases.Results All patients were available at the final follow-up,the mean duration was 16 months(range,12~24 months).At the last examination,the curative effect revealed excellent in 6 patients,good in 12 patients,fair in 2 patients and poor in 2 patients according to Neer function assessment system,the excellent and good rate was 81.8%.Conclusion Majority of treatment results of type C distal femoral fractures are improved through following rational rules of operation,applying appropriate internal fixation according to type of fractures,the reduction of articular surface and collapse fraction,strongly internal fixation,the proper exercise after operation.
