腮腺嗜酸性淋巴肉芽肿的超声诊断研究

目的探讨超声诊断腮腺嗜酸性淋巴肉芽肿的依据,旨在提高其诊断符合率。方法回顾性分析经手术、穿刺活检及病理证实的22例腮腺嗜酸性淋巴肉芽肿的二维声像图及彩色多普勒血流显像特点。从肿块的大小、个数、质地、境界和内部回声等指标做综合分析。结果超声诊断为嗜酸性淋巴肉芽肿12例,淋巴结肿大5例,低回声占位5例。超声诊断符合病理学为54%,超声诊断良性肿块符合率为100%。结论超声诊断应根据腮腺嗜酸性淋巴肉芽肿声像图特点,并结合临床特征和实验室检查,可提高诊断正确率。同时,超声检查对临床放射性治疗该疾病的疗效及预后的判断有很大的帮助。

Ultrasonographic diagnosis of eosinophilous lymphoid granuloma in the parotid gland

Objective To discuss the ultrasonographic features of eosinophilous lymphoid granuloma(ELG) in the parotid gland. Methods Gray scale and color Doppler ultrasonography of 22 ELG of the parotid gland confirmed by pathology post surgery or biopsy were analyzed retrospectively. The analyzed index included the size,number,texture,borderline and echoginicity of the lesion. Results The diagnosis of ultrasonography was 12 cases of ELG, 5 cases of swelling lymph nodes and other 5 lesions of hypoechogenicity. The diagnostic accuracy of ultrasonography was 54% compared with pathology. The differential diagnosis of benign or malignant lesions of ultrasonography was 100%. Conclusion The diagnostic accuracy of ultrasonography will be improved based on the features of ELG in the parotid gland and the combination of clinical characteristics and laboratory examinations.
