计算机辅助立体定向脑内靶点定位治疗帕金森病

摘    要:目的研究计算机辅助立体定向脑内靶点定位与术中电生理刺激结合确定丘脑腹外侧核(VL)、苍白球腹后外侧部(VPLP)在治疗帕金森病(PD)中的作用。方法对58例PD病人共进行76次手术,先进行立体定向CT扫描,将扫描资料在专用的计算机中确定解剖学靶点后,术中进行电生理刺激的方法验证和纠偏,根据调整后的结果进行射频毁损。结果立体定向CT扫描计算机定位准确率达92.1%,电生理纠偏率为7.9%。结论立体定向CT扫描计算机定位准确性很高,但术中电生理刺激验证和纠偏仍有必要。

Abstract:Objective To evatuate the effect of the computer assistant stereotactic operation combined with the electrophysiological-stimulation in thalamotomy and pallidotomy to treat the parkinson disease (PD).Method58cases of PD were operated for76times.The stereotactic CT-scanning and localization by computer assistant stereotaxis were performed,then to validate and correct any errors for the radio-frequency treatment.Result The correct rate of the stereotactic CT-scan-ning is92.1%,and the rate of rectify deviation is7.9%.Con clusion The correct rate of localization by computer assistant sereotaxis is heigh,but electrophysiological-stimulation for correct the targeting is necessary.
