CT血管造影在脑血管畸形诊断中的临床应用

摘    要:目的研究多层螺旋CT血管造影及其处理技术在脑动静脉畸形(AVM)方面的应用及诊断价值。方法20例患者行多层螺旋CT血管造影(MSCTA)检查,评估CT血管造影(CTA)在脑血管畸形诊断中的价值。除3例外其余患者均行数字减影血管造影(DSA)检查,并与CTA结果对照。结果20例患者的MSCTA断面和重建图像上均见到典型的畸形血管团,19例发现粗大的供血动脉,16例明确的引流静脉或扩大的静脉窦,6例病人合并亚急性出血.2例有出血后脑软化。结论MSCTA是脑血管病诊断的一种简单、快速、非侵袭和可靠的方法。MSCTA对脑动静脉畸形有重要的诊断价值。MSCTA在描绘脑血管三维结构以及毗邻骨结构上优于DSA。

Abstract:Objective To study the diagnostic value of MSCTA and the reconstruction for AVM.Methods 20 patients were detected by MSCTA,to explore the diagnostic value of CTA for AVM.All except 3 cases also underwent digital subtraction angiography(DSA).Results Typical malformated vessed masses were found on MSCTA.In all 20 cases,19 cases demonstrated dilated feeding ateries.Draining veins and enlarged venous sinus were seen in 16 cases,6 cases had the subacute hemorrhage.Conclusion CTA is a simple,rapid,noninvasive and reliable method for diagnosis of cerebrovascular diseases,which is especially useful in the preoperative planning,intraoperative orientation and long-term follow-up.MSCTA is playing an important role in the diagnosis of cerebral AVM.CTA is superior to DSA for depicting the three-dimensional anatomy of cerebral vasculature and adjacent bone structure.
