肺动静脉畸形的MR表现

摘    要:目的 评价MR在诊断肺动静脉畸形中的作用和价值。方法 回顾性分析 7例经肺动脉造影证实的肺动静脉畸形患者的MR表现 ,并与DSA比较 ,观察评价成像质量及显示病变血管的能力。结果 MR及CE MRA显示病灶为瘤样扩张的血管 ,有一条供血动脉和一条或数条引流静脉与肺门相连。对比增强肺动脉MR成像 (CE MRA)有助于显示病变全貌 ,图像质量堪与肺动脉造影相比。结论 CE MRA作为一种无创性评价大血管疾病的方法 ,不仅可以清晰地显示病变 ,而且能评价其血流动力学改变及预后 ,可望在一定程度上取代DSA。

Abstract:Objective To evaluate the effect of MR in diagnosed on pulmonary arteriovenous malformation (PAVM). Methods MR and CE MRA of 7 patients with PAVM confirmed by pulmonary artery angiography were analyzed retrospectively. Results MR and CE MRA showed PAVM was consisted of abnormal angiectasis, a feeding artery and one or several draining veins which connected to the hilar region. The shortage of rule MR sequence may be remedied by contrast enhanced MR pulmonary angiography which could show the full view of PAVM about the lesions, such as the angiectasis, feeding artery and draining vein. The quality of the image was as good as pulmonary angiography. Conclusion In the evaluation of PAVM, contrast enhanced MR pulmonary angiography could not only show the angiectasis and its feeding artery and draining vein, but also evaluate blood dynamics and prognosis. It may replace PA DSA in a certain extent as a kind of noninvasive technique.
