TCT、阴道镜和LEEP在慢性宫颈炎诊治中的探讨

目的:探讨TCT、阴道镜和LEEP在慢性宫颈炎诊治中的作用。方法:回顾性分析TCT为NILM 129例和阴道镜下活检为慢性宫颈炎404例行LEEP治疗患者的临床资料。结果:TCT为NILM 129例,9例术后升级,TCT阴性预测值93.02%。阴道镜下活检为慢性宫颈炎404例,术后升级4例,术前后断符合为400例(99.01%)。TCT与阴道镜活检诊断为慢性宫颈炎和LEEP术后病理符合率相比较有显著性差异(P<0.0001)。结论:TCT是筛查宫颈癌的一种较好的方法,阴道镜下活检准确率高,可以避免宫颈癌的漏诊。

Investigate the Effect of TCT , Colposcope and LEEP in the Diagnosis and Treatment for Chronic Cervicitis

Objective:To investigate the effect of TCT,colposcope and LEEP in the diagnosis and treatment for chronic cervicitis.Methods: 129 cases diagnosed Negative for Intraepithelial Lesion or Malignancy by TCT and treated with cervical loop electrosurgical excision procedure(LEEP) shortly after,were retrospectively studied.Results: The negative predictive value of TCT is 93.02%.In the pathology outcome Of the 129 patients,120 cases had chronic cervicitis,2 CINⅡ,3 CIN Ⅲ and 2 squamous carcinoma of the cervix 404 cases with chronic cervicitis diagnosed by colposcopially directed biopsy,4 cases were upgrade,400 cases had consistent pathologic diagnosis between preoperative and postoperative(99.01%).To compare the coincidence rate of diagnosing chronic cervicitis with colposcopially directed biopsy and TCT,there were significant discrepancy(P<0.0001).Conclusion: TCT is the better methods for cervical cancer screening,but the accurate rate of colposcopially directed biopsy is so high that missed diagnosis of cervical cancer can be avoided.
