cTnI、CK-MB在AMI早期诊断中的临床应用

摘    要:目的 :观察急性心肌梗死 (Acutemyocardialinfarction ,AMI)患者早期诊断指标心肌肌钙蛋白I(cardiactrofoninI,cTnI)与肌酸激酶同工酶MB(CreatineKinase MB ,CK MB)的水平 ,为AMI的诊断、预后及疗效判断提供可靠的参考依据。方法 :选用AMI患者 82例 ,不稳定性心绞痛 (UnstableAngina,UA)患者 30例 ,稳定性心绞痛患者 (StableAngina,SA) 2 2例。所有血清标本均采用化学发光免疫分析技术在Bayer公司的ACS:180系统上检测cTnI、CK MB的含量 ,并比较两者诊断AMI的准确性以及在体内的时间动态变化。结果 :AMI组患者血清中cTnI和CK MB同时增高 ,在 3组患者中cTnI诊断AMI的准确性为98.8% ,CK MB的准确性为 82 .8% ,两者有显著性差异 (P <0 .0 5 )。CTnI血清水平在第 4天仍维持在正常值以上 ,而CK MB在第三天就已全部恢复正常 ,cTnI持续时间较CK MB长 (P <0 .0 5 )。结论 :cTnI和CK MB联合应用可以提高AMI的诊断率 ,减少假阳性 ,对于AMI的病情变化也有一定的预示作用

Abstract:Objective: To observe the level of cardiac trofonin I (cTnI)and Creatine Kinase MB(CK MB) in the early stage of Acute myocardial infarction(AMI). Method: Serum was collected from 82 patients of AMI, 30 patients of unstable angina(UA), and 22 patients of stable angina(SA). The level of cTnI and CK MB in the serum was detected by the means of chemiluminescent immuneassay. We compared veracity of the two markers and time veraeity curve was observed.Results: cTnI and CK MB were all elevated in AMI patients. Among three group, the veracity of cTnI in diagnose AMI is higher than CK MB( P <0.05). Respectively the level of cTnI in the fourth day is still unnormal, the level of CK MB in the third day was all fall normal. The persistence time of cTnI is longer than CK MB ( P <0.05).Conclusion: The combination plot of cTnI and CK MB can improve the diagnostic rate of AMI.
