连续性护理干预对老年慢性心力衰竭患者病情控制和自我护理能力的影响

目的探讨连续性护理干预对老年慢性心力衰竭患者病情控制和自我护理能力的影响。方法选取住院治疗的慢性心力衰竭的患者110例，随机分为干预组和对照组，每组55例。对照组患者接受常规循环内科护理，干预组患者出院后接受6个月内的定期电话随访及门诊随访，比较两组患者再住院率和天数、死亡率和自我护理评分。结果干预组患者再住院率明显低于对照组（P〈0．05），再住院次数和总天数均明显少于对照组（均P〈0．01），患者自我护理能力明显高于对照组（P〈0．01），死亡率两组间无明显差异。结论连续性护理干预可以有效地控制老年慢性心力衰竭病情的发展，提高自我护理能力。

The effect of continuity of nursing intervention on controlling elderly patients with chronic heart failure and their self- care

Objective To explore effect of the continuity of nursing intervention on controlling elderly patients with chronic heart failure. Methods A total of 110 chronic heart failure patients in our hospital between January 2012 and December 2012 were divided into in- tervention and control groups, 55 cases in each group. The control group patients were received the conventional loop medical care, the inter- vention group patients were received regular telephone follow - up and outpatient follow - up within six months. After 6 months, the readmis- sion rate, number of day, mortality and self- care scores of the two groups were analyzed. Results The readmission rate, number of read- mission and total number of days of intervention group were lower than those of control group （X2 = 4. 475, t = 2. 689, -2. 872） ; the ability of patients＇self-care was better than that of control group （t = 3. 346 ） ; there was no statistical differences in mortality rate between the two groups （X2 = 0. 853） . Conclusions The continuity of nursing intervention can effectively control the development of elderly patients with chronic heart failure, improve the ability of patients＇self- care.
