内扎外剥保留齿线后侧切术治疗环状混合痔疗效观察

目的观察内扎外剥保留齿线后侧切术治疗环状混合痔的疗效。方法将90例环状混合痔患者随机分为治疗组和对照组各45例,治疗组采用内扎外剥保留齿线后侧切术治疗,对照组采用分段外剥内扎术治疗,比较2组近期疗效和远期疗效。结果 2组近期疗效无显著性差异;治疗组创面愈合时间短于对照组,术后疼痛、水肿、便血、尿潴留、肛门失禁等发生率低于对照组;治疗组远期复发、皮赘残留、肛门狭窄发生率低于对照组。结论内扎外剥保留齿线后侧切术治疗环状混合痔创面愈合时间短,术后并发症少,远期疗效满意,值得进一步研究。

Curative effect observation of internal ligation and outer stripping of reserving dentate line posterior side incision operation for circumferential mixed hemorrhoids

Objective It is to observe the curative effect of internal ligation and outer stripping of reserving dentate line posterior side incision operation on postoperative circumferential mixed hemorrhoids.Methods Ninety cases of circular mixed hemorrhoids were randomly divided into treatment group and control group with 45 cases in each one.The treatment group was treated with internal ligation and outer stripping of reserving dentate line posterior side incision operation,and the control group was treated with subsection external dissection and internal ligation operation.The short-term and long-term therapeutic effects were compared between both groups.Results There was no significant difference in short-term effect between the two groups.The wound healing time was shorter in treatment group than that in control group.The incident rates of pain,edema,hemafecia,urinary retention and anal incontinence were lower in treatment group than that in control group.The incident rates of long term recurrence,skin tags residue and anal stenosis were lower in treatment group than that in control group.Conclusion Internal ligation and outer stripping of reserving dentate line posterior side incision operation in the treatment of circumferential mixed hemorrhoids can shorten the wound healing time and decrease the incident rate of postoperative complications with a satisfactory long-term efficacy,so this operation is worth further study.
