经皮空心钉线缆张力带治疗髌骨骨折的临床研究

摘    要:目的探讨经皮空心钉线缆张力带治疗髌骨骨折的临床疗效。方法克拉玛依市中心医院骨科2006年2月-2011年1月共收治22例髌骨骨折患者,男性15例,女性7例,均为闭合性骨折。按Rockwood分型:Ⅱ型10例,Ⅲ型8例,Ⅳ型4例。均于透视下复位,经皮植入空心螺钉,钉尾捆扎不可吸收高强度骨科线,经皮于髌前行"8"字张力带固定。术后早期开始功能锻炼。结果 22例患者术后均得到8~24个月随访,术后8w时均达到临床愈合标准,平均11.5w获得骨性愈合。膝关节屈曲0~140度。无骨折再移位、感染、内固定断裂等并发症发生。按Bostman髌骨骨折功能评定标准评价:优17例,良5例,优良率为100%。结论经皮空心钉线缆张力带治疗髌骨骨折创伤小,避免了多种并发症,值得临床应用推广。

Abstract:Objective To explore the clinical effect of percutaneous cannulated lag screws and tension band with suture in treatment of patella fracture.Methods 22 patients with patella fracture were treated with percutaneous cannulated screws and tension band with suture.The healing of the fracture and recovery of the function of knee were investigated.Results Followed-up period was from 8 to 24 months.According to the Bostman outcome criteria,17 were excellent,5 good,the excellent and good rate was 100%.Neither displacement of the fractures,loosening,breakage of the internal fixation,nor such complications as infection occurred.Conclusion The cannulated lag screws and tension band with suture system for patella fractures is consistent with the principle of biomechanics internal fixation,which is less invasive and fewer complications.It is worth promoting.
