术后早期炎性肠梗阻的诊断与治疗

目的 探讨术后早期炎性肠梗阻的特点和诊断，治疗原则。方法 分析近期经治的术后早期炎性肠梗阻23例。结果 23例病人均经胃肠减压，应用生长抑素，肾上腺皮质激素及肠外营养等治疗后痊愈，平均治愈时间13.2天，无一例再手术引起；（3）症状以腹胀为主，腹痛相对较轻；（4）很少发生肠绞窄。（5）最好先试行保守治疗2-4周，多数有效。

The diagnosis and treatment of early postoperative inflammatory small bowel obstruction

Objective To investigate the principles of diagnosis and treatment of early postoperative inflammatory of small bowel obstruction (EPISBO) . Methods The clinical features and diagnosis procedures of EPISBO were analysed. Results 23 cases were all cured in the mean period of 13.2 days through gastroenteral decompression, somatostatin, dexamethasone,other drugs and TPN . There was no reoperations. Conclusion The time of EPISBO occures in two weeks. The clinical features are as follows: (1) occurring in early stage usually 3 to 7 days postoperatively; (2) most cases are caused by extensive adhesion resulted from aseptic inflammations; (3) abdominal distension is more prominent than abdominal pain; (4) Rare strangulated intestine; (5)conservative treatment is the first choice for EPISBO for 2 to 4 weeks, conservative therapy usually is effecdve.
