腔镜下高频超声刀甲状腺部分切除术围术期的炎性及应激性指标变化研究

目的探讨腔镜下高频超声刀甲状腺部分切除术围术期的炎性及应激性指标变化情况。方法选取2010年1月-2011年5月于本院进行腔镜下高频超声刀甲状腺部分切除术治疗的25例患者为观察组，同期采用直视下小切口手术进行治疗的25例患者为对照组，比较两组术前及术后3d、7d的炎性及应激性指标。结果观察组术后3d及7d的血清hs-CRP、TNF—α、IL－6及IL－8均低于对照组，而SOD、TAC及NO均高于对照组，而MDA则低于对照组，且观察组术后7d优于术后3d，P均〈0．05，有显著性差异。结论腔镜下高频超声刀甲状腺部分切除术围术期的炎性及应激性指标变化幅度小于直视下小切口手术。

Study on the change of the inflammatory and stress indexes of patients with partial thyroidectomy by endoscope and high frequency ultrasound knife during perioperative period

Objective To study the change of the inflammatory and stress indexes of patients with partial thyroidectomy by endoscope and high frequency ultrasound knife during perioperative period. Methods 25 patients with partial thyroidectomy by endoscope and high frequency ultrasound knife in our hospital from January 2010 to May 2011 were selected as observation group, 25 patients with small incision operation under direct vision at the same time were selected as control group, then the inflammatory and stress indexes of two groups before and after the operation at third and seventh day were compared. Results The serum hs-CRP, TNF-α, IL-6 and IL-8 of observation group after the operation at third and seventh day were all lower than those of control group, serum SOD, TAC and NO were all higher than those of control group, while the serum MDA were lower than those of control group, those indexes of observation group after the operation at seventh day were better than those after the operation at third day, all P〈0.05, there were all significant differences. Conclusion The change range of inflammatory and stress indexes of patients with partial thyroideetomy by endoscope and high frequency ultrasound knife during perioperative period are smaller than those of small incision operation under direct vision.
