侵袭性牙周炎患者龈下菌斑中的疱疹病毒与牙周致病菌

目的:检测侵袭性牙周炎(aggressive periodontitis, AgP)患者龈下菌斑中的人巨细胞病毒(human cytomegalovirus, HCMV)、EB病毒1型(Epstein-Barr virus.1,EBV一1),探讨疱疹病毒与牙周致病菌及牙周临床指标间的关系.方法:应用巢式PCR技术,检测89名AgP患者和31名健康对照者龈下菌斑中的2种疱疹病毒:人巨细胞病毒(HCMV)、EB病毒1型(EBV-1),同时应用16S rRNA为基础的PCR技术检测8种牙周可疑致病菌,包括伴放线放线杆菌(Actinobacillus actinomycetemcomitans, Aa)、牙龈卟啉单胞菌(Porphyromonas gingivalis, Pg)、福赛类杆菌(Bacteroides forsythus, Bf;现已更名为Tannerella forsythensis, Tf)、中间普氏菌(Prevotella intermedia, Pi)、直形弯曲菌(Campylobacter rectus, Cr)、具核梭杆菌(Fusobacterium nucleatum, Fn)、牙密螺旋体(Treponema denticola, Td)、变黑普氏菌(Prevotella nigrescens, Pn).结果:89名AgP患者的HCMV检出率为43.8%,显著高于健康对照者(12.9%),差异有统计学意义(P<0.01).AgP组的EBV-1阳性率虽高于对照组,但差异无统计学意义(P>0.05).检出6～8种牙周致病菌的AgP患者其HCMV和EBV-1检出率均明显高于仅检出3～5种细菌的AgP患者的病毒检出率(P<0.05).结论:HCMV、EBV-1在AgP患者龈下的检出率较健康对照高;且在检出牙周致病菌种类数较多的样本中,疱疹病毒的检出率高,提示牙周感染HCMV和EBV-1与侵袭性牙周炎相关,疱疹病毒与牙周致病菌在侵袭性牙周炎的发病中可能起着协同作用.
