发热门诊在传染性非典型性肺炎诊治中的作用

目的：初步探讨发热门诊在传染性非典型性肺炎(简称非典)诊断、治疗和管理中的作用。方法：回顾分析我院建立发热门诊的基本经过、运作情况及其在非典诊治和管理中所发挥的作用。结果：发热门诊成立后在2003年2月6日至2003年6月6日共接诊各类发热病例6824例次，初步诊断或疑似诊断为非典共334例，其中转入非典隔离观察病区140例(其中30例后转入隔离治疗区)，转入非典隔离治疗病区224例。我院所有确诊为非典的108例均由发热门诊(急诊)首诊并收入院治疗。发热门诊当值医师、护士未发生受非典病毒感染的情况，未发现对非典病例的漏诊现象。结论：发热门诊的建立有利于对发热病例中罹患非典的病人进行快速、有效的筛检和诊断。发热门诊的建立有利于对非典患者进行隔离、医学观察、治疗的安全治疗通道的建立。发热门诊详细、较全面的个人资料登记便于对非典患者进行相关的流行病学调查和管理。

The Effects of Fever Clinics in Diagnosis and Treatment of Infectious Atypical Pneumonia

Objective: To discuss the effects of fever clinics in diagnosis treatment and management of infectious atypical pneumonia (IAP). Methods: To analyze retrospectively the establishment, working of our fever clinic and its effects in diagnosis treatment and management of IAP. Results: From 6th February, 2003 to 6th June, 2003 after our fever clinic open, total 6824 patients suffered fever were accepted by us. 334 of the patients were diagnosed IAP or uncertain IAP, 140 of them were isolated in IAP observation ward (30 of which were transmitted in IAP isolation ward) and 224 of them were transmitted in IAP isolation ward. All of 108 IAP patients diagnosed by our hospital were firstly found and hospitalized at fever clinic,no one was missed. None of the duty doctors and nurses in fever clinic were suffered with IAP. Conclusions: The establishment of fever clinic is good for the rapidly effective screening and diagnosis of IAP patient from the people suffered fever. Its establishment is beneficial to the isolation, medical observation and treatment of IAP. The detail and complete personal information registered at fever clinic is easy to investigate and manage the IAP patients.
