经阴道子宫肌瘤剔除术的临床应用

目的探讨经阴道子宫肌瘤剔除术的临床特点及临床价值。方法对近2年我院收治的180例子宫肌瘤患者,随机分为经阴道和经腹手术组各90例,观察手术时间、术中出血量、剔除肌瘤数目、术后疼痛程度、体温和住院时间。结果经阴道组术中无一例中转开腹,无一例术中损伤膀胱、直肠。经阴道组与同期经腹组比较：2组患者的手术时间、术中出血量、剔除肌瘤数目无明显差异,经阴道组有对腹腔侵扰少、恢复快、痛苦小、住院时间短等优点。结论阴道子宫肌瘤剔除术术后的并发症少,对患者的创伤少,体表不留疤痕,利用女性生理腔道进行手术,更符合微创观念,而且手术的技巧易掌握,无需昂贵的设备,经济效益好。而对多发（≥5个）、巨大或盆腔有严重粘连的肌瘤首选开腹肌瘤剔除术。

Clinical Application Of Transvaginal Myomectomy

Objective To investigate the clinical characteristics and clinical experiences of transvaginal myomectomy. Methods With the 180 cases of uterine fibroids admitted in our hospital for the last 2 years, 90 eases of transvaginal operations and 90 cases of are divided randomly.The observation includes： bleeding during the operational time, volume, the number and impact of hurricanes struck off myoma, myoma weight, postoperative pain, body temperature and length of stay. Results In the ease of transvaginal operations, there＇s no conversions of intraoperative, and no injury of the bladder and the rectum, Compare with the abdominal operations, the operation time, bleeding, the removal quantities share no obvious differences. In contrast, transvaginal operations advances in less pain towards harassment, recovering fast, and short hospital stay.Conclusion With the complications of transvaginal myomectomy, there are less trauma patients, the body surface leaves no scars. The transvaginal myomectomy is more in accord with women＇s digestive tract for surgery, more in line with minimally invasive concept , and surgical techniques, easy--to--grasp without the need for expensive equipment, and economic benefits.And in the case of more myomas（≥5）, giant or pelvic adhesions myoma, the transvaginal myomectomy is more preferred
