非脱垂子宫经阴道切除术的临床应用及体会

目的对比非脱垂子宫经阴道切除与经腹式切除两种术式的优劣。方法将良性病变的非脱垂子宫切除的患者,分为TVH组与腹式手术组进行临床观察与对比。结果TVH组术中平均出血量,肛门排气时间,术后疼痛度,住院时间与经腹式手术组比较差异有统计学意义(p均<0.01);平均手术时间差异无统计学意义(p均>0.05)。结论TVH组子宫切除较腹式子宫切除具有对腹腔干扰少,术后胃肠功能恢复快,平均出血量少,住院时间短,术后疼痛度低,腹壁不留切口疤痕等优点,如果掌握好手术适应症及手术操作熟练,非脱垂子宫经阴道切除较经腹式手术有诸多优越性。

Non-prolapsed Uterus Transvaginal Excision of the Clinical Application and Experience

Objective To compare the advantages and disadvantages between transvaginal hysterectomy and transabdominal hysterectomy. Methods Patients with benign uterine diseasese were divided into TVH （transvaginal hysterectomy） group and TAH （transabdominal hysterectomy） group for clinic observing and comparing. Results There were significant differences （p 〈 0. 01 ） in the average bleeding volume during operation, the time of bowel function recurrence, the pain and hospital stay after operation between TVH group and TAH group. There was no statistical significant difference （p 〉0. 05 ） in the duration of operation between these two groups. Conclusion TVH group has advantages of minimal invasion, rapid recovery of gastrointestinal function, less average bleeding volume, less pain and shorter hospital stay after operation and no incision sear on abdominal wall. Having determined the surgical indications of operation appropriately and operated skillfully, transvaginal hysterectomy has more advantages than transabdominal hysterectomy.
