早期应用他汀类药对脓毒症患者炎性因子及预后的影响

目的:观察早期应用他汀类药对脓毒症患者肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)水平及预后的影响。方法:86例脓毒症患者符合入选标准,依据诊断脓毒症之前是否应用过他汀类药,分为非他汀组(44例)与他汀组(42例)。观察2组治疗前和治疗后28d血TNF-α、IL-6、hs-CRP水平变化,机械通气时间及28d死亡率。结果:2组治疗后血TNF-α、IL-6、hs-CRP水平较治疗前明显降低(P<0.01),其中他汀组下降更明显(P<0.01);2组机械通气时间及28d死亡率比较差异有统计学意义(P<0.05)。结论:早期应用他汀类药能改善脓毒症患者预后,他汀类药对脓毒症可能有一定的保护性作用。

Effect of Prior Therapy with Statin on Inflammatory Factor and Prognosis in Patients with Sepsis

OBJECTIVE： To observe the effect of prior therapy of statins on serum levels of TNF-α, IL-6, hs-CRP and prognosis in patients with sepsis. METHODS： 86 eligible patients with sepsis were divided into non-statin group （44 cases） and statin group （42 cases） according to the application of statin. The serum levels of TNF-α, IL-6 and hs-CRP, mechanical ventilation time and 28-day mortality were measured before and four weeks after treatment. RESULTS： The serum levels of TNF-α, IL-6 and hs-CRP were significantly decreased after treatment in both groups （P〈0.01）, especially in the statin group （P〈0.01）. There were significant difference in mechanical ventilation time and 28-day mortality between two groups （P〈0.05）. CONCLUSION： Prior therapy with statins can improve the prognosis in patients with sepsis, and have a role in the primary prevention of sepsis.
