膝关节置换术后类风湿关节炎患者应对方式、社会支持与生存质量的相关研究

目的:了解膝关节置换术后类风湿关节炎患者的生存质量,探讨其与应对方式及社会支持之间的关系。方法:采用世界卫生组织生存质量测定量表简表(WHOQOL-BREF)、医学应对问卷(MCMQ)及领悟社会支持量表(PSSS),对60例膝关节置换术后类风湿关节炎患者进行问卷调查,并用SPSS11.5软件包对数据进行统计分析。结果:和一般人群相比本组患者生存质量各维度得分普遍降低;采用的应对方式分别为面对(17.44±2.21)分、回避(17.42±2.64)分和屈从(11.62±1.19)分;患者领悟社会支持总分、家庭支持、朋友支持、其他支持得分分别为(54.64±12.48)分、(19.64±9.21)分、(14.78±5.42)分、(19.43±5.32)分。生存质量各维度得分与医学应对方式中的屈服应对得分呈负相关;生存质量各维度得分与社会支持总分及各因子得分呈正相关;同时社会支持总分及各因子得分与屈服应对得分呈负相关。结论:在临床工作中,医护工作者应转变工作模式,更多从生理、心理、社会多角度去关心患者,帮助他们采取积极的应对方式,充分利用社会支持,从而促进患者疾病的康复和生存质量的提高。

Study on the relationship between coping styles,social support and quality of life in patients who underwent knee joint replacement

Objective To investigate the quality of life(QOL) in patients underwent knee joint replacement and explore its relationship with coping style and social support.MethodTotally 60 patients who underwent knee joint replacement were assessed with WHOQOL-BREF,MCMQ and PSSS.Statistics analyses were performed using the Statistical Package for Social Science(SPSS) 11.5.ResultsThe score of QOL in those patients,comparing with the general population,was decreased generally.The coping styles were confrontation(17.44±2.21),avoidance(17.42±2.64) and resignation(11.62±1.19).The score of total PSSS,in family support,friends support and other support were(54.64±12.48),(19.64±9.21),(14.78±5.42),(19.43±5.32),respectively.The correlation ores of resignation coping style.Every dimension of QOL were positively correlated with total score of the PSSS.Meanwhile,the total score of PSSS and it′s two factors were negatively related to resignation coping style.ConclusionMedical staff should take care of patients from physical,psychological and social aspects during daily clinical practice.In order to improve QOL,it is essential to reinforce patients′ use opositive coping styles and help them take full advantage of social support.
