摘 要: | 目的探讨辛伐他汀对急性脑梗死患者血脂、抵抗素、脂联素水平及颈动脉内膜—中层厚度(IMT)的影响。方法将急性脑梗死伴颈动脉硬化患者113例随机分为观察组(56例)和对照组(57例),对照组给予基础治疗,观察组在此基础上加用辛伐他汀40 mg口服,疗程均为3个月。2组于治疗前、治疗后检测血脂、抵抗素、脂联素及颈动脉内膜—中层厚度变化。结果与治疗前比较,2组治疗3个月后总胆醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、抵抗素水平、脑卒中量表(NIHSS)评分及颈动脉内膜—中层厚度均降低,且观察组较对照组降低更明显(P均<0.05);高密度脂蛋白胆固醇(HDL-C)、脂联素水平均升高,且观察组较对照组升高更明显(P<0.05)。相关性分析表明IMT与TC、TG、LDL-C抵抗素呈正相关(r=0.18、0.24、0.16、0.21,P均<0.05),而与脂联素、HDL-C呈负相关(r=-0.26、-0.23,P均<0.05)。结论他汀类药物治疗能有效降低急性脑梗死患者的血脂及抵抗素水平,有效延缓和逆转动脉粥样硬化斑块,减少急性脑梗死的发生。

Abstract: | Objective To investigate the effect of simvastatin on serum lipids, resistin,adiponectin and carotid artery intima media thickness in patients with acute cerebral infarction. Methods 113 cases of acute cerebral infarction with carotid atherosclerosis were randomly divided into experimental group (56 cases) and control group (57 cases). Control group received basic treatment, observation group based on the use of simvastatin 40 mg orally, treatment lasted three months. Before and three mouth after treatment, the serum blood lipids, resistin, adiponectin were tested and carotid artery intima media thickness (IMT) were examined. Results After 3 months of treatment of two groups of total cholesterol ( TC), glycerin three greases (TG), low density lipoprotein (LDL-C), NIHSS, resistin and carotid artery intima media thickness decreased, the experimental group than in the control group decreased significantly ( P 〈 0.05), high density hpoprotein cholesterol ( HDL-C) adiponectin levels were elevated. The experimental group increased more obviously than in the control group ( P 〈 0.05 ). Correlation analysis showed that IMT and TC, TG, LDL-C resistin levels were positively correlated ( r = 0.18, O. 24, 0.16, 0.21, all P 〈 0.05), with adiponectin, HDL-C was negatively correlated ( r = - 0.26, - O. 23, all P 〈 0.05). Conclusion Statin therapy can effectively reduce the acute cerebral infarction patients with blood lipid and resistin level, effectively de- lay and reverse the progression of atherosclerotic plaques, reduce the occurrence of acute cerebral infarction. |
