兔冠状动脉微栓塞后血清肌钙蛋白与血液动力学变化相关性分析及通心络保护作用观察

摘    要:目的探讨兔冠状动脉微栓塞(CME)后血清肌钙蛋白Ⅰ与血液动力学的关系以及通心络的保护作用。方法日本大耳白兔40只随机分为假手术组、CME组、联合组(通心络+阿司匹林+阿托伐他汀)、非联合组(阿司匹林+阿托伐他汀)。采用酶联荧光分析法检测术前和术后6 h的cTnI数值,颈动脉插管连续观察血液动力学变化。结果 (1)血清cTnI水平:与术前比较,手术后假手术组无显著变化,CME组、联合组、非联合组均显著增高(P<0.05),联合组与非联合组比较差异亦有统计学意义(P<0.05)。(2)血液动力学变化:与术前比较,手术后假手术组仅左室收缩压(LVSP)明显降低,CME组、联合组、非联合组各项指标均降低(P<0.05,P<0.01),与CME组比较,联合组、非联合组均增高(P<0.05,P<0.01),且联合组、非联合组比较差异亦有统计学意义(P<0.01)。(3)相关性分析:除联合组-dp/dtmax外,各组LVSP、-dp/dtmax、dp/dtmax均与血清cTnI水平呈负相关(P<0.05,P<0.01)。结论阿司匹林、阿托伐他汀联合通心络可减轻兔冠状动脉微栓塞后心功能损害。

Correlation analyze of serum troponin with hemodynamic changes and the protective effect of Tongxinluo after coronary micro-embolization in rats

Abstract:Objective To investigate the relationship between cTnl and hemodynamic changes and the protective effect of Tongxinuo after coronary micro-embolization(CME) in rats model.Methods 40 Japanese white rabbits randomly divided into sham operation group,CME group,combination Tongxinluo group(Tongxinluo + aspirin + atorvastatin ),noncombination group(Aspirin + atorvastatin ).ELISA assay using fluorescence were applied to analyze preoperative and postoperative cTnI values for 6 h,continuous observation of carotid artery hemodynamics was also applied.Results(1) Serum cTnI is significant negative correlation with hemodynamic(P 0.05).Combination Tongxinluo further reduced myocardial injury after CME(P 0.05).(2) Hemodynamic changes;compared with the preoperative,sham surgery group only left ventricular systolic pressure(LVSP) decreased significantly,CME group,the joint group,non-union group,the indicators are lower(P 0.05,P 0.01),compared with the CME group,combination group,non-combination group were significantly higher(P 0.05,P 0.01),and the combined group,non-union group difference was also statistically significant (P 0.0I).(3) Correlation analysis:in addition to the combined group -dp/dt_(max),each group of LVSP,-dp/dt_(max),dp/ dt_(max) and serum cTnI levels were negatively correlated(P 0.05,P 0.01).Conclusion Aspirin,atorvastatin combined with Tongxinluo reduces rabbit ventricular function after coronary microembolization damage.
