摘    要:目的:探讨16层螺旋CT多平面重建及三维重建技术在平战骨骼创伤中的应用价值。方法:对2148例军地伤病员的2859个部位应用16层螺旋CT进行扫描,采集原始轴面图像数据,层厚采用0.625~1.25mm,利用工作站软件行多平面重建法(MPR)、容积再现法(VR)重建,最佳角度显示病变。结果:2859个部位中,骨折2175个,脱位684个,其中2834个三维图像可清晰显示骨折及脱位的整体情况,对10个肋骨及15个鼻骨裂隙骨折三维图像不能完全明确,需结合MPR图像后确诊。结论:16层螺旋CT多平面重建与三维重建技术相结合可以明显提高平战创伤中骨折及脱位诊断的准确率,为临床判断伤情及手术治疗提供更多有用的信息。

Abstract:Objective To evaluate the value of 16-slice spiral CT for the diagnosis of bone trauma by multiplanar reconstruction and three-dimensional reconstruction at peacetime and wartime.Methods 2 148 military and local cases were scanned by 16-row multi-slice CT.Totally 2 859 different parts were imaged.After gathering the primitive axial image data and reconstruction with the level thick 0.625-1.25mm,images were proceeded with MPR,volume rendering(VR)by workstation software,and best-rank angle were chosen to show pathological changes.Results Totally 2 859 different parts were scanned by 16-row MSCT and 2 175 parts were diagnosed as bone fracture and the other 684 parts as dislocation.Of the total,2 834 parts of bone fractures and the dislocations were clearly displayed and 10 parts of rib and 15 parts of nasal bone could not be clearly displayed by 3D reconstruction.After combining MPR images these obscure fracture parts were also diagnosed.Conclusion Diagnostic accuracy of bone fracture and dislocation are enhanced through combining MPR and 3D reconstruction techniques by 16-slice spiral CT at peacetime and wartime,which provides useful information for clinical judgment and treatment.
