二维超声与弹性成像技术在甲状腺癌诊断中的价值

摘    要:目的:探讨二维超声与弹性成像技术在甲状腺癌诊断中的价值。方法:回顾性分析经手术病理证实的66例81个恶性病灶的二维声像图及超声弹性图表现;二维超声诊断甲状腺癌的指标包括病灶大小、边界、形态、纵横比、内部回声、钙化类型、周边声晕、有无后方衰减等;同时分析81个甲状腺恶性病灶的超声弹性图表现,将图像分为0~Ⅳ级,并与病理结果作对比。结果:根据二维超声诊断指标,将可疑恶性病灶归入恶性组内,二维声像图与病理符合率74.07%;将二维声像图与弹性图联合诊断,则联合诊断指标与病理符合率86.42%,二者比较差异有统计学意义(P0.05)。结论:二维超声和弹性成像技术综合判断肿瘤良恶性,明显提高了超声对甲状腺癌诊断的准确性。

Abstract:Objective:To explore the value of two-dimensional ultrasonography combined with elastography in the diagnosis of thyroid cancer.Methods:66 patients,with a total of 81 malignant thyroid lesions(all confirmed by surgery and pathology),were enrolled in this study.The two-dimensional ultrasonography and elastograms of all lesions were retrospectively analyzed.Two-dimensional ultrasonic indicators of thyroid cancer included size,border,shape,vertical and horizontal ratio,internal echo,calcification type,halo,with or without attenuation.To analyze the elastograms of these 81 malignant lesions,the images were divided into 0~Ⅳ levels.The ultrasonic findings were compared with postoperative pathology.Results:According to two-dimensional indicators,the suspicious malignant lesions classified as malignant group.The consistent rate of two-dimensional ultrasonography and pathology was 74.07%.To combine two-dimensional ultrasonography and elastography,the consistent rate of the combined diagnostic index and pathology was 86.42%.There was significant difference between two methods(P0.05).Conclusion:If we combine two-dimensional ultrasonography and elastography to differentiate malignant tumors from benign ones,the diagnostic accuracy of ultrasonography in thyroid cancer can be markedly improved.
