甲状腺功能亢进性心脏病的临床和X线表现分析(附56例报告)

对５６ 例甲状腺功能亢进性心脏病回顾性分析。结果表明，Ｘ 线上心脏增大占７８ ．８ ％ ，多数为轻度增大，占６４ ．３ ％ ，心脏增大形态中以二尖瓣型多见，占８３ ．７ ％ ，少数表现为普大—二尖瓣型、普大型及主动脉型。２８ ．５ ％ 病例甲状腺功能亢进的症状、体征不典型，临床易误诊。认为甲状腺功能亢进性心脏病Ｘ 线诊断要密切结合临床资料

Clinical and X-ray manifestation in patients with hyperthyroid cardiopathy: retrospective analysis of 56 cases

The hyperthyroid cardiopathy from 56 patients were retrospectively analyzed. Results showed that the cardiac enlargement in X-ray films was 78.8%, 64.3% of them were slight enlargements. 83.7% of cardiac enlargements were categorized as mitral type. The few were normal-mitral type, normal type, and aortic type. 28.5% patients had untypical symptoms and signs of hyperthyroid and were easily diagnosed. Therefore the X-ray in diagnosing of hyperthyroid cardiopathy should be closely combined with clinical manifestation. Key words hyperthyroidism; heart diseases; X-rays ohistochemistry method.A multivariate stepwise Cox's proportional hazard model analysis was performed to evaluate the prognostic value of the number of AgNORs,the area of AgNORs,the expression of PCNA,p53 and the Dukes′ grading.The formula was showed as fellowing.pv=e((0.710238X l+0.9185X 2+0.527513X 3+0.463156X 4),when Ln(pv)≥12.0,the patient was infered that he(she) couldn't survive over 5 year.When Ln(pv)<12.0,the patient was infered that he(she) could survive over 5 year.The efficienty was 91.6%,the sencitivity was 90.3%,the specificity was 93.1%.These results indicated that the AgNORs quantity and the expression of PCNA and p53 might be helpful to evaluate the prognosis of the patient with colorectal cancer.
