经导管持续闭式引流联合大剂量IL-2和顺铂治疗恶性胸腔积液

目的观察经导管持续闭式引流联合大剂量IL-2和顺铂治疗恶性胸腔积液的近期疗效和毒副反应。方法32例恶性胸腔积液患者,先使用美国Arrow公司生产的一次性单腔中心静脉导管进行胸膜腔穿刺置管和闭式引流胸水,待胸水流尽后再经导管予胸腔内注药,每次IL-2200~300IU,顺铂60~80mg,每周1次,连续2~4周,1个月后观察疗效和不良反应。结果CR23例,PR5例,NC2例,PD2例,总有效率为86.5%(28/32)。结论经导管持续闭式引流联合大剂量IL-2和顺铂治疗恶性胸腔积液疗效肯定,是控制恶性胸腔积液的有效方法。

Intracatheter injection of IL-2 and DDP combined with closed thoracic drainage in treatment of malignant pleural effusion

Objective To evaluate the responses and adverse reactions of in section of IL-2 and DDP combined with closed thoracic drainage to treat malignant pleural effusion. Methods Thirty-two patients with malignant pleural effusion were applied with smugly Lumen central venous catheters made by Arrow Company. All patients were received the closed thoracic drainage and then,were treated by intracavity injection of IL-2 and DDP. IL-2 was given on dosage of 2 000 ~ 3 000 IU each time and DDP at 60~80 mg each time. The duration of the treatment was one time per week for a consecutive two to four weeks. The therapeutic effects and adverse reactions were observed one month after the treatment. Results There were 23 patients of CR,5 patients of PR,2 patients of NC and 2 patients of PD. The overall effective rate for the treatment was 87.5% (28/32). Conclusion After draining pleural fluid completely,intracavity injection of IL-2 and DDP is effective in treating malignant pleural effusion.
