颈动脉扭曲的彩色多普勒超声诊断

摘    要:目的探讨颈动脉扭曲的彩色多普勒超声显像特征及诊断价值。方法32例颈动脉扭曲患者,应用彩色多普勒超声观察颈动脉走行情况并测量颈动脉内径及内中膜厚度(IMT),显示颈动脉扭曲时的血流情况并测量颈动脉收缩期峰速度(PSV)及阻力指数(RI),检测结果与对侧(无扭曲侧)颈动脉进行对照。结果颈动脉扭曲时表现为颈动脉走行呈“S”形或“C”形弯曲,IMT增厚。彩色多普勒显示血管腔内形成方向各异的涡流。与对侧无扭曲颈动脉比较,颈动脉扭曲时内径增宽,斑块发生率、PSV及RI增高。结论颈动脉扭曲多发生于动脉硬化基础上并伴有血流动力学改变,彩色多普勒超声能简便有效诊断颈动脉扭曲。

Abstract:Objective To study the ultrasonic visualization feature and diagnostic value of color Doppler ultrasound on carotid distortion. Method The anatomical ran way, inner diamete, intima-media thickness (IMT),blood flow state, peak systolic velocity (PSV) and resistent index (RI) of tortuous carotid of 32 patients with carotid distortion were acquired by color Doppler ultrasound. Results Carotid distortions in two dimensions manifested curving forms of"S"or"C" and increasing of IMT. The color Doppler imaging showed the directions of blood stream had changed and shaped into vertex flow. Compared with contralateral normal carotid, tortuous carotid showed increased inner diameter, PSV and RI simultaneously with high incidence of plaque (P〈0.01 or P〈0.05). Conclusions base of arterial sclerosis and follow with hemodynamic change. The color Doppler Carotid distortion usually occur on the ultrasound can provide an efficacious and convenient method for diagnosising carotid distortion.
