针刺配合胞磷胆碱钠片治疗面神经炎疗效观察

目的观察针刺配合胞磷胆碱钠片治疗面神经炎的临床疗效。方法 80例面神经炎患者随机分为对照组和观察组,每组40例。对照组采用胞磷胆碱钠片联合甲钴胺片、维生素B1治疗,观察组在对照组基础上配合针刺治疗。观察两组治疗前后House-Braekmann(H-B)分级、体征评分和外周血中神经生长因子(NGF)、脑源性神经营养因子(BDNF)、肿瘤坏死因子?(TNF-?)、白介素-6(IL-6)水平变化,并比较临床疗效。结果对照组总有效率为77.5%,观察组为90.0%,两组比较差异有统计学意义(P<0.05)。观察组治疗后H-B分级优于对照组(P<0.05)。两组治疗后各项体征评分与同组治疗前比较,差异有统计学意义(P<0.01);观察组治疗后的各项体征评分低于对照组(P<0.01)。两组治疗后外周血中NGF、BDNF的水平较同组治疗前显著升高,TNF-?和IL-6水平较同组治疗前显著下降(P<0.01);观察组治疗后NGF、BDNF、TNF-?和IL-6水平与对照组比较,差异有统计学意义(P<0.01)。结论针刺配合胞磷胆碱钠片治疗面神经炎疗效显著,作用机制与神经营养因子表达增高,炎症因子表达降低有关。

Observations on the Efficacy of Acupuncture plus Citicoline Sodium Tablets in Treating Facial Neuritis

Objective To investigate the clinical efficacy of acupuncture plus citicoline sodium tablets in treating facial neuritis.Method Eighty patients with facial neuritis were randomized to control and observation groups,with 40 cases in each group.The control group was treated with citicoline sodium tablets plus mecobalamin and vitamin B1 and the observation group,with acupuncture in addition.The House-Braekmann(H-B)grade and the sign score were recorded and peripheral blood nerve growth factor(NGF),brain-derived neurotrophic factor(BDNF),tumor necrosis factorα(TNF-?)and interleukin-6(IL-6)levels were measured in the two groups before and after treatment.The clinical therapeutic effects were compared.Result After treatment,the House-Braekmann(H-B)grade was lower in the observation group than in the control group(P<0.05).There were statistically significant pre-/post-treatment differences in the sign scores in the two groups(P<0.01).Post-treatment sign scores were lower in the observation group than in the control group(P<0.01).Peripheral blood NGF and BDNF levels increased significantly and TNF-?and IL-6 levels decreased significantly in the two groups after treatment compared with before(P<0.01).After treatment,there were statistically significant differences in NGF,BDNF,TNF-?and IL-6 levels between the observation and control groups(P<0.01).Conclusion Acupuncture plus citicoline sodium tablets has a marked therapeutic effect on facial neuritis.Its mechanism of action be related to an increase in the expression of neurotrophic factor and a decrease in the expression of inflammatory factors.
