针刺列缺穴治疗紧张性头痛疗效观察

目的观察针刺列缺穴治疗紧张性头痛的临床疗效,并通过经颅多普勒超声(TCD)观察其对颅内血流动力学的影响。方法 60例紧张性头痛患者随机分为观察组和对照组,每组30例。对照组给予尼莫地平口服,观察组在对照组基础上给予针刺列缺穴治疗。采用TCD观察两组治疗前后大脑前动脉(ACA)、大脑中动脉(MCA)和大脑后动脉(PCA)的血流速度变化,观察两组治疗前后头痛程度变化,并比较两组临床疗效。结果两组治疗后头痛评分、头痛发作次数、头痛指数均低于同组治疗前(P<0.05),观察组低于对照组(P<0.05)。观察组治疗后ACA、MCA、PCA的舒张末期最大血流速度(Vd)、平均血流速度(Vm)均高于治疗前(P<0.05),对照组治疗后ACA、MCA、PCA的Vd均高于治疗前(P<0.05)。观察组治疗后ACA的Vd、Vm均高于对照组(P<0.05),MCA、PCA的Vm高于对照组(P<0.05)。观察组总有效率为90.0%,高于对照组的66.7%(P<0.05)。结论针刺列缺穴可有效改善紧张性头痛患者的颅内血流速度,控制或减轻临床症状,疗效显著。

Observations on the Therapeutic Effect of Point Lieque Acupuncture on Tension Headache

Objective To investigate the clinical efficacy of acupuncture at point Lieque in treating tension headache and observe its effect on intracranial blood flow using transcranial Doppler(TCD).Method Sixty patients with tension headache were randomized to observation and control groups,with 30 cases in each group.The control group received oral administration of nimodipine and the observation group,acupuncture at point Lieque in addition.Blood flow velocity in the anterior cerebral artery(ACA),middle cerebral artery(MCA)and posterior cerebral artery(PCA)was measured using TCD in the two groups before and after treatment.Headache severity was assessed in the two groups before and after treatment.The clinical therapeutic effects were compared between the two groups.Result The headache score,the frequency of attacks and the headache index were lower in the two groups after treatment than before(P<0.05)and they were lower in the observation group than in the control group after treatment(P<0.05).Vd and Vm in ACA,MCA and PCA were higher in the observation group after treatment than before(P<0.05).Vd in ACA,MCA and PCA was higher in the control group after treatment than before(P<0.05).After treatment,Vd and Vm in ACA and Vm in MCA and PCA were higher in the observation group than in the control group(P<0.05).The total efficacy rate was 90.0%in the observation group,which was higher than 66.7%in the control group(P<0.05).Conclusion Acupuncture at point Lieque can effectively improve intracranial blood flow velocity,control or relieve the clinical symptoms and produce a marked therapeutic effect in patients with tension headache.
