MDM2及P53在儿童急性淋巴细胞白血病中的表达及临床意义探讨

用免疫组化的方法检测了23例初诊急性淋巴细胞白血病患儿骨髓细胞MDM2及P53的表达，结果发现：MDM2的阳性表达占56％（13/23），而且化疗2周末，MDM2阳性组10例患者仅4例获完全缓解，而MDM2阴性组8例患者7例获完全缓解。P53阳性表达率30％（7/23）。MDM2与P53共同表达者，无1例在化疗2周末完全缓解，两者同时阴性者在化疗2周末均达完全缓解。提示，急性淋巴细胞白血病患儿MDM2的表达特点是伴P53的表达预示着化疗效果差。该两项指标的检测有助于选择治疗方案及预后的判断。

Expression of MDM2 and P53 in Pediatric Acute Lymphoblastic Leukemia and Its Clinical Significance

MDM 2 and P53 expression of bone marrow cells from 23 pediatric patients with acute lymphoblastic leukemia (ALL) at initial diagnosis was detected by using immunohistochemical assay. The result showed that the positive rate of MDM 2 protein expression was 56 % (13/23). Only 4 cases achieved CR following two week chemotherapy in 10 MDM 2 positive cases, but 7 cases get CR after two week chemotherapy in 8 MDM 2 negative cases. The positive rate of P53 protein expression was 30 % (7/23). All patients in MDM 2 P53 positive cases failed to achieve CR after two week chemotherapy. All patients get CR in MDM 2 P53 negative cases after two week chemotherapy. These results indicate that MDM 2 expression, especially accompanied with P53 expression in the pediatric patients with ALL, predict a poor prognosis of chemotherapy.
