儿童食道异物并发症的临床分析

目的探讨食道异物对儿童的影响,评估儿童食道异物的治疗策略。方法对我院从2006年5月～2011年5月收治的137例儿童食道异物患者进行回顾性分析,包括发病时间、异物性质、形状及手术方式等。结果 126例经手术处理,10例随粪便排出,1例拒绝治疗。14例出现并发症,包括1例死亡,1例食管穿孔,2例食管周围炎,10例食管黏膜损伤。经抗生素、鼻饲胃管保守治疗,术后住院时间从3 h～21 d,平均3.28 d。结论注意主动脉弓对食管的影响,早诊断、早治疗是降低儿童食道异物并发症及死亡率的关键。

Clinical analysis of complications with children esophageal foreign body

OBJECTIVE To investigate the effect of esophageal foreign body on children and assess the treatment strategies of esophageal foreign body in children.METHODS The clinical data of 137 children with esophageal foreign body treated in our hospital from May,2006 to May,2011 were retrospectively analyzed.The duration period of the disease,the nature and shape of the foreign body,surgical methods were recorded.RESULTS The foreign bodies were removed though esophagoscopy in 126 cases,discharged along with the bowel movement in 10 cases.One patient refused treatment.Postoperative complications occurred in 14 cases,including one death,one case of esophageal perforation,2 cases of esophageal surrounding inflammation,and 10 cases of esophageal mucosal injury.A few patients need antibiotic and nasogastric tube after operation.The postoperative hospital stay time was 3 hours to 21 days（average 3.28 days）.CONCLUSION Early diagnosis and treatment are key to reduce the postoperative complications and mortality of esophageal foreign body in children.The aortic arch should be pay attention to during operation.
