双蒂腓肠肌肌皮瓣治疗小腿下1/3软组织缺损

因无适当的组织移转复盖,故小腿下1/3软组织缺损较难修复。报告用双蒂腓肠肌肌皮瓣转移修复小腿胫前下1/3软组织缺损4例,临床效果良好。双蒂腓肠肌肌皮瓣具有血运丰富,有利于炎症吸收和骨折愈合等优点。并讨论了该皮瓣的设计原理和临床应用。

Bipedicle Gastrocnemius Musculocutaneous Flap for Defects of the Distal One-third of the Leg

The lower one-third of the leg consists of a tight skin directly cove- ring bone and musculotendinous stuctures. Soft tissue defects in this area are difficult to repair. However the method of using bipedicle gastro- cnemius musculocutaneous flap to repir soft tissue defect on the lower one-third of the leg is good. The reported 4 cases of open fracture of the leg with soft tissue defect had been treated with this method and had obtained satisfactory results. The plenty circulation from the proxi- mal end of the flap allows the distal pedicle to become narrow and therefore be ahle to advance without jeopardizing the blood supply. This paper discussed the principles of designing the flap and its clinical ap- plication.
