声门型喉癌术后放射治疗与单纯手术疗效的比较

目的　探讨声门型喉癌手术治疗加用术后放疗能否比单纯手术提高治愈率。方法　应用随机分组的方法进行临床试验。初治的声门型喉癌T2～ 4 ,年龄在 72岁以下 ,经过临床检查确定解剖分型、TNM分期 (1978UICC)及手术类型后 ,随机将患者分为单纯手术组 (SA组 )和综合治疗组 (RS组 )进行治疗。结果　共完成治疗 92例 ,SA组 60例 ,RS组 3 2例。SA组 3生存率 79 0 % ,5年生存率74 9% ,RS组 3年生存率 76 3 % ,5年生存率 72 0 % ,无显著差异 (χ2 =2 .5 ,P =0 .1)。结论　声门型喉癌术后放射治疗量 40Gy不能提高生存率

Surgery plus Postoperative Radiation vs. Operation alone for Glottic Carcinoma

Objective To study the validity of postoperative radiation in improving the curative rate of glottic carcinoma.Methods A prospective randomized clinical trial was conducted with surgery plus postoperative radiation(RS)as one group and surgery alone (SA) as the other group.The age of patients was less than 72 years,and the operative types were decided after clinical examination.The patients were randomly divided into SA and RS groups.Dose of postoperative irradiation was 40 Gy.Results Ninety-two patients with glottic carcinoma were treated,60 in the SA group,32 in the RS group.All patients were follwed up for over three years.In SA group,the 3-year survival rate was 79.0%,the 5-year survival rate 74.9%,In RS group,3-year survival rate was 76.3%,the 5-year survival rate 72.0%.There is no significance statistically in surival rates between the two groups ( P =0.1).Conclusions Postoperative radiotherapy with 40 Gy didn't increase the survival rate of glottic carcinoma.
