阵发性心房颤动患者心功能与P波离散度的关系

目的观察阵发性心房颤动(PAF)患者心功能与P波离散度(Pd)的关系,探讨心功能指标对PAF的预测价值。方法用CDFI超声测定38例PAF患者和30例正常组的P波最大时间(Pmax)、P波离散度(Pd)。结果PAF组Pmax和Pd阳性率明显高于与对照组;Pmax+Pd联合使用,特异度和阳性预测值明显高于单用者;PAF组左心房内径、左房舒张末期面积、左房收缩面积较正常对照组明显增大(P<0.05);左房峰值充盈率较正常对照组明显减小(P<0.05);左室峰值充盈率(LvPFR)与LAPER呈正相关(r分别为0.62,P<0.01);Pd与左室射血分数(LvEF)呈负相关(r=-0.5678,P<0.05)。结论Pd与Pmax结合使用,具有更高的预测价值。

Relationship between heart function and P wave dispersion(Pb) in patients with paroxysmal atria fibrilation

Objective To understand relationship between heart function and P wave dispersion(Pb) in patients with paroxysmal atrial fibrillation(PAF).Methods Compared with a normal group of 30 individuals,the blood flow of the portal vein in 38 cases with paroxysmal fibrillation was examined with color Doppler flow.Results The positive rate of maximun P ware interval(Pmax) and P wave dispersion(Pd) in PAF were obviously higher than that in control group;The prognosticate of specificity and positive in alliance group were obviously higher than that in single group;The left atrium diameter(LAD),left atrium end diastolic phase area(LAEDA) and left atrium end shrink phase area(LAESA) in PAF were obviously higher than that in control group;The left atrium peak value rate(LAPER) in PAF were obviously lower than that in control group;The comparision between left ventricle peak value full rate(Lv PFR)and LA PER was positine relationship(r=0.62,P<0.01);The comparision between Pd and left ventricle eject fraction(LvEF) was negative relationship(r=-0.5678,P<0.05).Conclusion The alliance determine of Pd and Pmax have estimate value in patients with paroxysmal atrial fibrillation.
