低分子量肝素治疗急性脑梗死的临床观察

目的:观察低分子量肝素(LMWH)治疗急性脑梗死的安全性和疗效。方法:对108例AIS病人随机分成LMWH组(38例),小剂量肝素(SDHU)组(36例),对照组(34例)。LMWH的用法为0.4mL皮下注射,1次/12h,小剂量肝素的用法为3250U,1次/12h,皮下注射;对照组为复方丹参20mL加入500mL生理盐水中静脉滴注,1次/d。3组均连续用药14d,治疗前后分别进行神经功能缺损评分,检查血常规、肝功能、肾功能、凝血功能。结果:LMWH组总有效率86.8％,SDUH组总有效率80.5％,都显著高于对照组的64.7％。LMWH组、SDUH组在出血性合并症及病死率与对照组无显著差异。结论:LMWH、SDUH治疗AIS安全、有效。

The Application Research on the Treatment of Acute Ischemic Stroke by Using Low Molecular Weight Heparins and Small Dose of Heparins

Objective: The study of the security and effect of the treatment of AIS by using low molecular weight heparins (LMWH) and small dose of heparins (SDUH) .Methods: 108 patients were randomly divided into 3 groups; The first groups (38 cases treated with LMWH) ,second group (36 cases treated with SDUH) and third group (34 cases,control) .The first group was injected 0.4mL of LMWH pypodermically,q12h.The second group was given hypodermic injection of 3250U of SDUH,q12h.The control group was given 20mL compound salvia mlihiorrhiza and 500mL physical salire by intravenous drip for once everyday. After 14 days of the continuous treatment, the impairment of nerves system function was judged and the routine blood count and functions of liver, kidney and thrombokinesis were tested and compared with that before treatment. Results: The first group (LMWH) efficiency was 86.8% .The second group (SDUH) was 80.5 % . Both of them were much higher than the control group, which was 64.7 % . The three groups were nearly the same in bleeding complication and fatality rate. Conclusion: LMWH and SDUH are secure and effective in treating AIS.
