多药耐药基因及P-糖蛋白在恶性淋巴瘤组织中的表达

摘    要:目的 观察多药耐药基因 (MDR -1)及P -糖蛋白 (P -gp)在恶性淋巴瘤组织中的表达情况 ,探讨恶性淋巴瘤的耐药机理。方法 应用RT -PCR法和免疫组织化学法检测 5 0例 ( 2 0例初治 ,30例复发 )恶性淋巴瘤患者MDR -1及P-gp的表达水平。 结果 初治患者MDR -1基因和P -gp表达较低 ,分别为 10 % ( 2 /2 0 )和 15 % ( 3/2 0 ) ;而复发患者MDR -1基因和P -gp表达明显增高 ,分别为 5 6 .7% ( 17/30 )和 40 .0 % ( 12 /30 ) ,两组比较差异有显著性 ( χ2 =11.0 9和 3.5 7,P <0 .0 5 )。 10例同时MDR -1基因和P -gp表达阳性 ,阳性符合率为 2 0 % ( 10 /5 0 )。 结论 经过化疗后的淋巴瘤患者 ,其肿瘤组织中的MDR -1基因和P -gp表达水平增高 ,说明该基因的表达不但与临床肿瘤在化疗中获得性抗药性有关 ,而且可能与天然抗药性有关。

Abstract:Objective To observe the expression of multi-drug-resistant gene (MDR-1) and P-glucoprotein (P-gp) in malignant lymphomatous tissues and the drug-resistant mechanism of malignant lymphoma. Methods A RT-PCR technique and an immunohistochemistry method were used to determine the expressive levels of MDR-1 and P-gp in 50 patients with malignant lymphoma (preliminary treated of 20 cases, recurrence of 30 cases). Results The expressive levels of MDR-1 and P-gp were low in preliminary treatment patients, and the levels were 10%(2/20) and 15%(3/20), respectively; but the levels of them increased in recurrent patients, and they were 56.7% (17/30) and 40.0% (12/30), respectively. The differences were statistically significant between these two groups, the values of χ 2 were 11.09 and 3.57, respectively, P<0.05. The expressions of MDR-1 and P-gp were positive in these 10 cases; the positive coincidence rate was 20% (10/50). Conclusions The levels of MDR-1 and P-gp in Patients increased after chemotherapy. The outcome proved that the expression of this gene in clinical tumor was not only related to the acquired drug-resistance during chemotherapy, but also might be related to the natural drug-resistance.
