以皮肤科疾病为首发症状的糖尿病40例临床分析

目的探讨以皮肤科疾病为首发症状的糖尿病的临床特点、诊断及误诊原因。方法回顾性分析以皮肤科疾病为首发症状的糖尿病40例患者的临床资料。结果 40例中首诊确诊为糖尿病32例,误诊、漏诊8例,误诊率20.0%,误诊、漏诊时间1 d～6个月。40例患者经对症治疗,糖尿病均得到控制,皮肤科疾病均得以治愈或好转。随诊6个月～1年,皮肤科疾病症状未见复发。结论对以皮肤科疾病为首发症状的非典型糖尿病症状的患者,特别是原因不明的皮肤反复感染及瘙痒者,应提高认识,详细询问病史,认真检查,避免误诊和漏诊。

Dermatological Diseases as the First Symptom of Diabetes： An Analysis of 40 Cases

Objective To analyze the clinical features, diagnosis and reasons for misdiagnosis in patients with dermatological diseases as the first symptom of diabetes. Methods Clinical data of 40 patients with dermatological diseases as the first symptom of diabetes were retrospectively analyzed. Results Among the 40 patients, diabetes was fi agnosis occurred in 8（20.0%）. The misdiagnosis rstly diagnosed in 32 and misdiagnosis and missed diand miss diagnosis time ranged from 1 day to 6 months. 40 patients with symptomatic treatment, diabetes was controlled, department of dermatology disease were cured or improved. The follow up of 6 months-1 years,no recurrence of the symptoms of the disease in department of dermatology. Conclusion The diagnosis of diabetes for patients with dermatological dis- eases as the first symptom, especially for those with unexplained recurrent skin infections and itching, depends on awareness-raising, detailed case history and careful examination to avoid misdiagnosis and missed diagnosis.
