两种营养方式应用于食管癌患者术后早期的效果比较

目的探讨肠内营养和肠外营养两种营养方式对食管癌患者术后恢复的影响。方法采用查随机数字表法,将96例患者分为肠内营养（enteral nutrition,EN）组49例及肠外营养（parenteral nutrition,PN）组47例,并分别进行不同营养支持,观察体重损失率、术后排便时间、7d下床活动患者比例、食欲正常患者比例、住院天数、营养费用、并发症;并观察术前、术后3d、术后7d血清总蛋白及白蛋白、淋巴细胞计数。结果EN组肠功能恢复较PN组早,平均住院时间少于PN组,营养费用明显低于PN组,两组间差异有统计学意义（P〈0.05）。体重损失率两组间比较无统计学意义（P〉0.05）。EN组术后7d能下床活动者和食欲正常者的比例高于PN组,差异有统计学意义（P〈0.05）。患者术前和术后3d,PN组与EN组血清白蛋白、总白蛋白、淋巴细胞计数的差异无统计学意义（P〉0.05）;术后7d,EN组血清白蛋白、血清总白蛋白、淋巴细胞计数高于PN组,差异有统计学意义（P〈0.05）。结论肠内营养较肠外营养能更快恢复肠道功能和体力,提高机体营养状态和免疫功能,对食管癌患者术后早期营养状态的改善起重要作用,更符合现代营养支持要求。
